Abstract: To map and synthesize the scientific literature on constrictive bronchiolitis and bronchiolitis obliterans, with a specific focus on the emerging association with e-cigarette use, encompassing histopathological features, chemical exposures, clinical outcomes, and management strategies across diverse etiological contexts. The review uses 46 references and builds its evidence map from 34 original studies with 1656 total participants/sample observations (topic-deduplicated ΣN). The evidence mapped here supports a biologically plausible and clinically observed but low-certainty link between e-cigarette aerosol inhalation and constrictive bronchiolitis, anchored by detection of diacetyl in 39 of 51 flavored products at concentrations up to 239 μg per e-cigarette and methylglyoxal in all tested JUUL pod flavors. Histopathologic confirmation of small airway-centered fibrosis with partial reversal after cessation, alongside a life-threatening adolescent case with persistent fixed airflow obstruction, indicates that vaping should be considered in the differential diagnosis alongside established occupational, transplant-related, and autoimmune causes of this disorder. Clinically, this highlights vaping cessation and exposure history-taking as immediate priorities in young patients presenting with unexplained fixed airflow obstruction. The dominant uncertainty remains the absence of prospective cohort data, and future research should establish longitudinal vaping cohorts with serial pulmonary function and imaging to define incidence, latency, and dose-response.
