What is this paper about
Endurant is not just another EVAR device story: this review shows where its long-term durability is real, and where it becomes fragile. The full read maps registry, hostile-neck, chimney, EndoAnchor, limb, renal, and late endoleak evidence to clarify a practical point for vascular specialists: Endurant success is driven less by deployability and more by proximal sealing quality, anatomy, and surveillance.

Abstract: To map and synthesize the available structured evidence on the Endurant stent graft, with emphasis on its clinical durability, safety, anatomical applicability, complication profile, adjunctive techniques, and emerging device-modification research in AAA and related aortoiliac pathology. The review uses 86 references and builds its evidence map from 133 original studies with 29309 total participants/sample observations (topic-deduplicated ΣN). The evidence indicates that the Endurant stent graft is a durable and broadly applicable EVAR platform, with registry data supporting 5-year freedom from aneurysm-related mortality near 97.8% and continued sac regression at 10-year follow-up, while outcome vulnerability concentrates in hostile proximal neck anatomy and outside-IFU use, where 5-year freedom from type IA endoleak fell to 89.4% versus 96.7% within IFU. This pattern highlights proximal sealing quality—rather than device feasibility alone—as the dominant determinant of long-term success. Adjunctive strategies such as EndoAnchors and careful chimney configuration appear to mitigate risk in selected complex anatomy, but supporting evidence remains largely observational. Future research should prioritize harmonized neck phenotyping and prospective comparative studies of planned adjunctive fixation in short-neck and hostile-neck subgroups to clarify when Endurant durability can be reliably preserved.
