Abstract: To synthesize the evidence from a scoping review on the long-term outcomes, surveillance modalities, risk stratification, and real-world adherence patterns of follow-up after endovascular aortic aneurysm repair (EVAR), and to map the landscape of complications that necessitate this lifelong monitoring. The review uses 119 references and builds its evidence map from 204 original studies with 688170 total participants/sample observations (topic-deduplicated ΣN). This scoping review indicates that the durability of EVAR is fundamentally tethered to lifelong surveillance, yet the real-world delivery of that surveillance is failing in ways that directly translate into excess mortality. The dominant signal is that patients lost to follow-up experience a four-fold higher 1-year mortality and 5-year survival of only 62% versus 84%, even as compliance with annual imaging falls to roughly 50% by 5 years. These findings support a shift toward risk-stratified protocols that de-escalate imaging after marked sac regression to ≤40 mm while intensifying monitoring for persistent endoleaks and hostile anatomy. Interpretation is constrained by the predominantly retrospective evidence base and heterogeneous surveillance definitions. Future work should prospectively validate personalized, AI-assisted surveillance pathways and test implementation strategies to close the compliance gap in high-risk subgroups.
